Ann Bacher, FNP-C, PMHNP-BC — founder of The Balanced Wellness — on the winding road that led her to primary care, and why continuity matters more than she was ever taught it did.

Ann Bacher's path to primary care didn't start in a clinic. It started in the Marine Corps — serving in crash/fire rescue, running toward emergencies that most people run from. A decision made young, for reasons that shaped everything that came after: discipline, service, and a taste for work that mattered.

After the Marines came nursing. And after nursing came the emergency room, where for years she learned to think fast, stay calm, and hold steady when the world around her was not. ER work teaches you to see a patient's whole life in fifteen minutes — the diabetes and the divorce and the depression all tangled together in one crisis. It also teaches you that fifteen minutes is never enough.

Two certifications, one philosophy

That gap is why she became a family nurse practitioner. Across more than 30 years in healthcare — from crash/fire rescue to the ER to primary care for adults and our older population — the pattern she kept seeing wasn't going to fix itself: patients whose bodies were treated by one clinician and whose minds by another, with neither one talking to the other, and neither one having enough time. So she added a second board certification in psychiatric mental health.

Today that means her primary care patients get something rare: one clinician who can manage the blood pressure, the antidepressant, and the conversation about what's actually going on at home — in the same visit, under the same membership — without pretending those are three separate problems.

Caring for our older population

The Balanced Wellness has a particular heart for older adults — people navigating complex medications, cognitive changes, grief, and the kind of loneliness that tends to be treated with more pills instead of more presence.

All care is delivered by live, face-to-face video — and the model is built for it: a consistent clinician who knows your history, medication regimens simplified rather than layered, and family kept in the loop. Nobody gets treated as a collection of separate problems.

Outside the clinic

Ann lives in Florida with her husband. She grew up around water and still does her best thinking near it. She reads more than she watches, keeps promises she makes to patients, and is unfailingly on time.
